What to Bring
Your medical history and surgery history
List of the medications, supplements, herbal products, and over-the-counter products you take
Outside images (on CD/disc or USB) — X-rays, MRI, CT scan, ultrasound
Written reports from the radiologist (description of what the imaging study showed)
Reports of injections/shots performed outside HSS
Reports of foot/ankle surgical procedures performed outside HSS
Orthotics, shoe inserts, or braces that you have been using
Preparing For Your Visit
What should I bring?
It is helpful to bring documents describing your prior treatments for your condition (physical therapy, bracing/orthotics, injections, medication, etc.), we will review these in your visit.
If you have orthotics and/or a brace for your foot/ankle, we recommend that you bring them to the appointment as well.
Similarly, if you have had an injection or a medication, it is helpful to bring in the report of what was given.
You can request records and imaging from your prior treatment provider (unfortunately, we are not allowed to request your records for you!) or medical records office at your prior healthcare facility. This will help your visit run smoothly and improve our understanding of your health so we can better tailor our treatment recommendations for you.
I’ve had imaging (XR, CT, MRI) before. Can you review that?
If you have prior imaging of your foot/ankle, please bring both the actual images (typically on a CD-ROM/disc or a USB/flash drive) and the radiology report. Contact the facility where it was performed to ensure that you get both the CD and the report (written description).
However, even if you have had imaging in the past, Dr. Henry may recommend to obtain additional or updated x-ray views at the time of your visit to help better diagnose and evaluate your problem and recommend treatment. Learn more about imaging here.
What will you ask?
Even if you have had prior treatment or if you have received a diagnosis/plan from another doctor, Dr. Henry still wants to hear about you: when your symptoms started, what triggered them, what makes your pain/symptoms worse/better, and treatments that you have tried. We ask these questions because we want to hear things from your perspective and how they affect your life.
You will also be asked about your medical and surgical history outside of your foot and ankle — this is important to understand how your foot/ankle affects your health (and vice versa), whether or not you undergo surgery.
What should I wear?
We recommend that you dress in loose, comfortable clothing bottoms for your visits so we can easily examine your feet, ankles, and legs. We can provide disposable shorts and/or a hospital gown if you need to change out of your tights/hosiery, skinny jeans, or other fashionable form-fitting options.
